Required Documentation:
- North Carolina Health Assessment - is required for all students K-12 entering NC schools for the first time.
- Meningococcal Conjugate Vaccine - BOOSTER dose required by age 17 or prior to entering 12th grade, whichever comes first OR Medical/Religious form must be completed to be exempt for vaccination.
- THIS INFORMATION MUST BE PROVIDED WITHIN 30 CALENDAR DAYS OF THE FIRST DAY OF SCHOOL ATTENDANCE. IF THESE REQUIREMENTS ARE NOT MET, YOUR CHILD MAY NOT ATTEND SCHOOL.
ONSLOW COUNTY SCHOOLS MEDICATION POLICY
Generally, medications should be given at home prior to the school day. However, Onslow County Schools staff understands some medications must be given during the school day; students with life-threatening medical conditions may require the administration of emergency medications while on school campuses. Thus, in accordance with Onslow County Schools, Board of Education Policy 6125, North Carolina General Statute 115C-375.1 and the North Carolina Board of Nursing, Nurse Practice Act:
- School staff cannot administer any medication, including emergency and over the counter medications, without a completed Onslow County Schools Medication Permission form; signed by the authorized licensed health care provider and the parent/guardian.
- The Onslow County Schools Medication Permission form must include student name, drug name, dose, route, frequency, prescriber name and contact information. The medication/pharmacy container must match the medication permission form and include the pharmacy name.
- Student health concerns and thus student medications are confidential according to the Federal Education Rights and Privacy Act (FERPA) therefore, student confidentiality and privacy must be protected during the medication administration process (National Association of School Nurses, 2020).
Contact your child’s school nurse for help managing newly diagnosed or chronic health conditions, medications at school, treatments or procedures at school, care planning for health conditions and assistance with health care referrals or resources.
Please note the following school health procedures as we work together to ensure a healthy, safe environment for all our students:
- Medications (prescription and over-the-counter) are not allowed at school unless an Onslow County Schools “Permission for Prescribed Medication to be Given During School Hours” form has been completed by both the parent and the doctor. The form and the medication must be brought to the school by an adult, students may not transport medication.
- Hearing, dental, and vision screenings are periodically done at the school. If there is a concern, you will be notified by the school nurse.
- Head checks will be conducted at school. If a student is sent home with head lice, they cannot return to school or ride the bus until they are rechecked at the school office.
- As per Onslow County Schools Board Policy (4230.5), students with a fever 100° or greater, with vomiting, or diarrhea will be sent home and may not return until they are free of symptoms for 24 hours (without medication) before they may return to school.
- If your child has a suspicious rash, they should be checked by your family physician before coming to school or riding the bus.
- It is essential that the nurse and the school have current emergency telephone numbers to ensure the parent/guardian will be notified in the event a student becomes ill or injured. Please update the school office whenever these numbers change.
